With Ian's help. taking the weight off, I'm starting to feel much better.
I've had a re-evaluation of my life in light of my stay in hospital, I was thinking I was maybe going to semi-retire at 60 (stop advertising the business, just service old customers and let it drift), its moved up from likely to almost certainly.

I don't blame you for choosing retirement. Having been self-employed myself I wouldn't want to be doing it now, there are too many people and organisations out there that seem to want to make life difficult. If the government wonders why we have one of the lowest productivity figures per head in Europe it should look at the obstacles thrown up by inefficiencies and incompetences in big organisations. Small businesses do really well in Germany because they get better support from big business.
